Probably the rubberized jaws, despite the seemingly sufficient clamping force, gradually move along the fan exhaust louvre until the weight of the whole holder+phone assembly exceeds the clamping force of the jaws. It was therefore a good idea to occasionally reseat the holder all the way to the exhaust stop and re-lock it for safety.
