Really flat.
The description said it had a spirit level for easy installation. It certainly doesn't have one, and thus it's pretty tricky to catch it straight, practically meaning throw a TV on it, measure it with a spirit level, take it off, and try to straighten it out.
The locking screw should come with someone to screw it in. It requires an extremely long screwdriver, and the screw just won't fit.
